This form may only be used by employees and former employees of Capilano University (or Board members) for the purposes of disclosing wrongdoing as defined by the Public Interest Disclosure Act (PIDA).

We encourage disclosers to provide their name and contact information but recognize that some individuals would rather remain anonymous. The completed form will be sent to the University's designated officers.

Employees who wish to make an anonymous disclosure without leaving contact details should be aware that the designated officer may be unable to seek clarification or further information, which may limit investigations and also means that we will not be able to provide you with information about the investigation process.

If you still wish to remain anonymous, please leave the "Contact Information" section blank and proceed to the section titled description of wrongdoing.
